Separation of Employment
IMPORTANT: An E-Form 3 provides notification from the department to Human Resources that the employee is separating from employment within your department. Upon receipt, the Benefits team will provide benefit-related details to the employee. Please ensure that the department processes the E-Form 3 timely.
The supervisor is responsible for completing and submitting the “Separation Notification” and completing the Exit Checklist no later than the employee’s last day of employment within your department.
