Separation of Employment
An E-Form 3 provides notification from the department to Human Resources that the employee is separating from employment within your department. Upon receipt, the Benefits team will provide benefit-related details to the employee.
The supervisor is responsible for completing and submitting the “Separation Notification” and completing the Exit Checklist no later than the employee’s last day of employment within your department.
