Separation of Employment
IMPORTANT: An E-Form 3 provides notification from the department to Human Resources that the employee is separating from the University. Upon receipt, the Benefits team will provide benefit-related details to the employee.
An E-form 3 is not required for an employee transferring out of your department to another department on campus.
Please ensure that the department processes the E-Form 3 timely.
The supervisor is responsible for completing and submitting the “Separation Notification” and completing the Exit Checklist no later than the employee’s last day of employment within your department, whether they are separating from the University or transferring to another department.
A “Frequently Asked Questions” is available for the employee leaving the University.
